REGISTRATION AS AN ANIMAL PHYSICAL THERAPIST

All applicants must meet the following criteria:

  1. Be of good moral character.
  2. You must be an active licensed physical therapist in this state for at least 1 year.
  3. You must be in good standing with the Nevada State Board of Physical Therapy Examiners.
  4. You must have successfully completed at least 100 hours of instruction or course work, or a combination of both, in the area of animal therapy, which must include, without limitation, assessment and planning of treatment, behavior, biomechanics, common orthopedic and neurological conditions, comparative anatomy, neurology, and therapeutic modalities and exercises; and
  5. You must have completed at least 125 hours of supervised clinical experience in animal physical therapy with a licensed veterinarian.

From each applicant the following will be required:
  1. A completed and signed application, notarized with a photo attached.
  2. A completed child support information form.
  3. Proof of licensure by the Nevada State Board of Physical Therapy Examiners stating that you have an active license and are in good standing.
  4. Proof that you have fulfilled the required course work (transcripts or certificate of completion) and proof of supervised clinical experience (signed off by a licensed veterinarian).
  5. A fee of $50.00. This fee covers the expense of application and licensing fees thru this calendar year, at which time you will receive a renewal form for the upcoming year.
